GENERAL AFFIDAVIT.
State of Tennessee County of Hamilton, SS:
In the matter of The Pension Claim of David Cooper, Co. E 4th Tenn Cav.
Personally came before me, a Notary Public in and for the aforesaid County, and State, Wm W Cooper aged 59 years citizen of the Town of Sherman Heights in the County of Hamilton and State of Tennessee, well known to me to be reputable and entitled to credit, and who being duly sworn declare in relation to the aforesaid case as follows:
The said Soldier David cooper had small pox at Nashville” Tenn in 1863 before I was enlisted in the regt
He also contracted cold at Nashville Tenn and on the Missippi River into Missippi which caused a breast or Lung trouble or disease from which he suffered more or less while in the service from 1863
Also I remember that said David Cooper contracted disease of liver and chills and fever at Decutar Alabama fall 1864 and was sent to hospital fall of 1864 at Nashville Tenn for treatment also he complained and was troubled with Rheumatisms at Nashville Tenn Winter 1864 and 1865 during the Hoall Campaign around Nashville and at East port Missippi in Gravelly Springs
I further declare that I have no interest in said case and are not concerned in its prosecution.
