DECLARATION FOR INVALID PENSION.

TO BE EXECUTED BEFORE A COURT OF RECORD OR SOME OFFICER THEREOF HAVING CUSTODY OF ITS SEAL.

On this 15th day of January A.D. one thousand and eighty 90 personally appeared before me, a Clerk of the Circuit Court, a court of record within and for the county of Bradley, State of Tennessee, who being duly sworn according to law. declares that he is the identical David G. Cooper who was ENROLLED on the 13th day of February, 1863 in Company E of the 4th Regiment of Tenn Cav commanded by C D Champion and was honorably discharged at Nashville Tenn on the 12th day of July 1865; that his personal description is as follows: Age, 45 years; height 5 feet 9 inches; complexion Fair; hair; Red; eyes, Blue. That while a member of the organization aforesaid, in the service and in the line of his duty at Nashville Tenn, in the State of Tennessee on or about the — day of Winter 1864, he Contracted cold which settled on his lungs and pneumonia fever And disease of liver contracted near Nashville Tenn winter and spring of 1864 And chills and fever at Decutar Ala summer 1864. Contracted diarrhea near Mobile Ala Spring of 1865 and Piles.

That he was treated in hospitals as follows: Hospitals No 9 Nashville Tenn winter 1863. Officers Hospital Nashville Tenn fall 1864 Hospital New Orleans La 1865 Gen USA Hospital Memphis Tenn spring 1865.

That he has never been employed in the military of naval service otherwise than as stated above_____________________________________________________________

At since leaving the service this applicant has resided in the ___ of Bradley Co in the State of Tennessee and his occupation has been that of a Farmer

That prior to his entry in the service above named he was a man of good, sound, physical health, being when enrolled a Farmer That he is now B/4 disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States.

 

He hereby appoints with full power of substitution and revocation Wm F. Duncan of Chatata Tenn. his true and lawful attorney to prosecute his claim. That has never received nor applied for a Pension. That his Post Office Address is Cleveland county of Bradley State of Tennessee.
