GENERAL AFFIDAVIT.
State of Tennessee County of Bradley SS:
In the matter of Minors NO. 704338 P. C. service of David G Cooper Co. “E” 4 Tennessee Cav. Vols. on the 12 day of March 1900 A.D. personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Dr. J. B. Shugart aged 49 years a resident of Cleveland in the County of Bradley and State of Tennessee, whose Post Office address is Cleveland ” , well known to me to be reputable and entitled to credit, and who being duly sworn declare in relation to the aforesaid case as follows:
Have practiced medicine for about 21 years.
I attended said Cooper the latter part of his last illness.
Was called Aug. 4 – 1899 and continued to visit him daily till the 19 – of same month and think he died the next day. – August 20 – 1899.
Gastro-eutenitis was the ?________? cause of death. He was a sufferer of chronic Gastro-Hepatic Catarrh with it coricouitant conditures. (Note: this part of the affidavit was handwritten by the physician, and was very hard to decipher)
I further declare that I have no interest in said case and are not concerned in its prosecution.
